The individual named below (referred to as “I” or “me”) desires to attend Trumpet Camp 2026 in the nation of Uganda, including travel to and from various site(s) within Uganda (the “Trip”), with Heritage Pastors Association, a division of a Christian Union, Inc., a non-profit religious organization (“CU”).

In consideration of being permitted by CU to participate in the Trip and in recognition of CU’s reliance hereon, I agree to all the terms and conditions set forth in this Release of Liability, Indemnification and Assumption of Risk Agreement (this “Release”).

1. I acknowledge that participating in the Trip entails both known and unknown risks that could result in illness, contraction of disease, severe physical and emotional injury, paralysis, death, or damage or loss of property. These risks include, but are not limited to: accidents in the course of travel to and from the destination; risk of injury or illness commonly associated with construction, recreation or other missionary service activities (including the risk of negligent acts or omissions); flight delays, cancellations or loss of luggage or personal items; and risks associated with vising, living and working in regions with underdeveloped security, buildings, roads, sanitation, food, water, and health care services and facilities. I understand that such risks are inherent to and cannot be eliminated from the Trip. I further understand that the Trip includes travel in countries and regions where there may be a risk of criminal or terrorist activity which may include, for example, risks robbery, theft, kidnapping, abduction and assault. NOTWITHSTANDING THESE RISKS, I ACKNOWLEDGE THAT I AM VOLUNTARILY PARTICIPATING IN THE TRIP WITH KNOWLEDGE OF THE DANGERS INVOLVED. I HEREBY AGREE TO ACCEPT AND ASSUME ALL RISKS ARISING FROM OR ASSOCIATED WITH THE TRIP, WHETHER CAUSED BY THE NEGLIGENCE OF CU OR OTHERWISE.

2. I confirm that I am in good health and proper physical condition and do not have any medical or other conditions that would impair my ability to participate in the Trip. I will comply with all applicable laws, orders, directives, and guidelines related to the Trip, while at the camp and conference site(s) and while traveling to and from the camp and conference site(s). I will also follow all instructions, recommendations, and cautions of CU at all times.

3. To the fullest extent provided by law, I, and on behalf of my heirs, representatives, executors, administrators, next of kin, successors and assigns, hereby agree not to sue and further agree to indemnify, defend, release, discharge and hold harmless CU and its members, directors, trustees, officers, employees and agents, and their respective successors and assigns (collectively, "Releasees") from and against all claims, both present and future, arising out of or in way connected with the Trip and/or any claim asserted with respect to any injury of person(s) or damage to property resulting from, or arising out of, participation in the Trip, except to those claims arising out of the recklessness or willful misconduct of CU or Releasees. I agree that if, despite this Release, I or any person on my behalf makes any claim against any of the Releasees falling within the scope of the release herein, I will indemnify and hold harmless all of the Releasees from and against such claim and any associated litigation expenses, attorneys’ fees, losses, liabilities damages or other costs any of them may incur.

4. I hereby consent to receive medical treatment deemed necessary if I am injured or require medical attention during my participation in the Trip. I understand and agree that I am solely responsible for all costs related to such medical treatment and any related medical transportation and/or evacuation. I hereby indemnify, defend, release, discharge and hold harmless CU and all other Releasees from any claim based on such treatment or other medical services.

5. Consistent with biblical commands (see Matthew 18:15-20, I Corinthians 6:1-8), any claim(s) and/or dispute(s) related to this Release that I and CU are unable to resolve via mutual discussion within 30 days shall be settled by mediation and, if necessary, legally binding arbitration under the Rules of Procedure for Christian Conciliation™ (found at https://www.aorhope.org/icc-rules) or such other alternative dispute resolution entity or person as I and CU may mutually agree in writing, which process will be commenced by both sides within 30 days of the conclusion of attempts to resolve the claim(s) and/or dispute(s) directly. I and CU agree that any arbitration award may be entered in any court having jurisdiction, and both expressly waive our right to file a lawsuit(s) or claim(s) against one another for such disputes, except to enforce an arbitration decision. Notwithstanding the foregoing, in the event of actual or threatened violation of this Agreement, CU may obtain from any court of competent jurisdiction such injunctive relief as may be appropriate to compel a mediation or arbitration process in accordance with the terms of this Section.

6. I have read this Release in its entirety, I understand its terms, and I enter into it knowingly, voluntarily and intentionally for the purpose of inducing CU to allow me to participate in the Trip. I am age 18 or older and am duly authorized to sign this Release on behalf of all parties listed below.
