I have read and understand the CU Code of Conduct and I agree to comply or I may be immediately removed as a Summer Project participant.

An invitation to fill out a reference form will be sent when your application is approved.

Travel Insurance Affidavit I am responsible for the purchase of proper travel health insurance for the duration of my trip with CU. I will send all travel insurance information to my CU Project Director for their files.

Signature Christian Union requires that you certify your application by submitting an electronic signature. To certify your application, read the text below and provide an electronic signature (type your name) and click Submit Application. “To the best of my knowledge, all of the information in this application is true and complete. I also authorize you to make such inquiries as may be necessary in arriving at an acceptance decision, which includes contacting the people completing my references, my local CU staff person, or others. I hereby release those persons from all liability to inquiries in connection with my application. If accepted, I understand that I will be asked to sign a safety waiver form and will be responsible for the cost of the project. I also understand that Christian Union will provide personal assistance if I choose to raise support for the project.”
